How to enroll in Ogden International School
Check the homepage first (Ogden may not be available depending on the area of residence, so check it. If you live in downtown, Ogden is the best)
Email to the person in charge: As of January 2022, email to the address of iquestell@cps.edu (listed on the website). Subject to change.
When you email, you will be instructed to make an appointment reservation after creating the required documents.
Preparation of required documents (Unlike Japan, it is difficult because there are many sheets. See below for details)
Other things you need to bring on the day of Appointment: Birth certificate (English translation of family register, passport of parents and person, 2 documents that can prove your address [Electricity bill receipt, apartment contract, insurance contract] , Documents received from social security office])

Bring the above documents with you on the date and time when you took the Appointment.
After confirming the documents, you will be notified by phone whether it is acceptable.
A CPS (Chicago public school) email address and Student ID will be issued within 2 days. Use it to create a CPS account
I received an email from my homeroom teacher by the day before school.
What to bring (varies greatly depending on grade / class): Crayons, pencils, glue sticks, water bottles, spare masks
Breakfast and lunch are served at school in principle, so don't worry, more than half of the children bring their lunches (because they serve typical American meals such as pizza, burgers, and nachos? ?).
First day of school: There is an explanation from the teacher in charge such as classroom guidance.
School day: Self- check your health every morning in advance (no need to submit). On the day we go to the local school, everyone starts going to school at around 8:15, and the gate opens at 8:25. Depending on the corona infection situation in the entire class / school, some or all students will be transferred to online classes (if vaccinated, in-person classes will be continued if there are no symptoms)
In the online class (Google Classroom + Google meet), the general meeting starts at 8:30 in the morning, and the class starts at 8:45.
Classes end at 15:15 for local schools and 15:30 online. In principle, a guardian must pick you up at the local school.
You can get a free corona antigen test at school every Tuesday.

PROOF OF SCHOOL DENTAL EXAMINATION FORM

It seems to be almost the same as page 15 here. Bring this to your dentist and ask your doctor to describe it. In dentistry, X-rays, cleaning, and fluoride application were performed. Fortunately, my son had no abnormalities such as tooth decay.
CERTIFICATE OF CHILD HEALTH EXAMINATION
Health certificate. When you were examined by Northwestern children's practice ( https://www.nwcppediatrics.com/ ), it was issued separately in this format (no need to bring it). You will be asked in detail about vaccination. It was said that it was helpful to bring the vaccination items on this health certificate along with the Mother and Child Handbook.
Ophthalmology examination.
The format is almost the same as page 16 of. Bring it with you when you go to an ophthalmologist and ask them to write it down. As many tests (including fundus examinations!) Were performed as when making glasses and contacts. My child found myopia and made glasses on the spot and started wearing them a few days later.
